Admission requirement:
Each child must visit the daycare for a half-hour before placement to help reduce separation fears.
If we agree that the child will be placed, the parents must provide the following information:
· Registration form 1 & 2
· Field trip form (supplied to parents at PPLC)
· Health inventory
· All about my child instruction form
· All about my child form
· Health inventory addendum
· Immunization certificate
· Two emergency cards
· Authorization to dispense medication
· Meal benefit form (supplied to parents at PPLC)
· Making a difference signature card (supplied to parents at PPLC)
· Contract (supplied to parents at PPLC)
